In this case presentation, Daniel Brown and Dr Oelsner will discuss the ongoing clinical work with a patient who has fluctuating homicidal and suicidal ideation, command hallucinations, and powerful paranoia. The presentation and discussion will focus on the destructive power of psychotic features, both manifest and hidden. Emphasis in this presentation will be on the development of the capacity to understand and work with psychotic experiences that are hidden from view of both patient and therapist. To this end, transference/counter-transference dynamics will be discussed as a foundational clinical tool that can be used to help further the work.
